Patients are becoming more digitally-savvy, relying on wearable and wireless devices to complement their care management, whether on their own or as part of post clinical or hospital care.
Health Storylines™ integrates with over 300 wearable and wireless devices including Fit Bit and Apple Health Kit.
Health Storylines™ and Health Storylines Creator™ will be used at the Palo Alto VA to support Wellness in Rehabilitation, an evidence-based pilot program designed to maximize recovery and function for patients during neurological, musculoskeletal, and cancer rehabilitation. Patients and caregivers in the program are assigned to groups which meet weekly for education and activities, such as exercise, cooking, mindfulness, and support sessions. Patients are encouraged to lead a healthy lifestyle and track their behaviors during the week, and each is given an iPad with Health Storylines™ software to assist with at-home monitoring and adherence. The software provides a number of key features to support the program, including patient education, surveys & logging, patient-provider messaging, messaging between group members, integration of wearable data, and monitoring of patient progress.
